I had chondromalacia patellae before I was diagnosed with Dystonia (although I clearly had it then in retrospect).My cartilage was cracking off and despite having been telling my consultant I couldnt play sports, run etc any more. I remember the first words he said after my arthroscopy-'you must have been playing contact sports!' (by then my walking was deteriorating fast and I'd not been able to play any sports for 5 years)
My knees didnt swell,go red or 'crack' (and crack isnt the right word-I could wake people in the next room with it!)for several weeks-then it came back. This was when I was 18 (half my life ao!) and it all soon came back. My knees are in such a state now-to the point where replacements have been mentioned (in fact they started mentioning it at 18) but I understand that because it's not a stable joint, the spasms would just pull the knee straight out. I do have sinificant hypermobility too, but reckon the spasms alone would do this. I also have problems with my hips but I think if they ever ot really bad, they would consider replacements as they are more stable
