hi greg. i also had it creep into the index of my fretting hand. in my case, it's been extrememly important and helpful to keep both hands separate while practicing to keep the "pathways" as pure and unconfused as possible. I like to relax the pick hand while slowly fretting the notes one at a time (only one finger on fretboard and strings at a time)and concentrating on the feeling of it in that finger. i hold it there till that weird dystonic weak feeling leaves. this reinforces the separtation of my fingers for me. likewise, when i exercise the pick hand, i'll relax the fret hand off of the guitar. this has helped me a lot.
