I believe that retraining is the only way to recovery.You mentioned SMR therapy and that is one kind of retraining.J.Farias says it too,retraining with slow and gentle movement exersises on the instrument.I got FD in dec 05,RH middle finger,and in 06 FD terminated my classical playing,but I have always played jazzguitar(plectrumstyle)as a kind of sidething and I got more involved in that kind of playing for awile.But the condition got worse and worse so I had to do something.In july/august 08 I started retraining and now 17/18 months later I'm much better.I can play small pieces and studies on the classical guitar if I'm a little bit careful.So I feel that I'm on the way to recovery.But it's gonne take a long time,maybe 2,3 or 4 years.I believe that it's possible to recover 100%.The thing is not to give up but keep on with the retraining program.
