i don't know...seems like your case is task specific (specifically any task that involves the index). lots of people here have problems other than just their instrument. some worse than others. i couldn't type, write, button a shirt, pick up objects, finger pick or flat pick for that matter. i'm amazed you can still hold a pick from your description...unless you're holding with other fingers. the forearm pain seems a little bit out of the ordinary, but that could be because of just how much you're tensing those muscles. like i said, i did have shoulder pain that i believe was directly associated with tension caused by dystonia. once i learned how to relax my muscles, that was the first thing to go away. i also had lots of tension in my forearm and i'm sure that if i kept going down the dystonic path i was on, i probably would have developed pain there as well.if i were you, i'd practice doing one of those other non-guitar tasks with a relaxed arm until you can do it without tension. i did this for 2 years. the great thing is that it benefited my trying to play guitar as well. i used to practice picking up small objects with my index and thumb by first completely relaxing my hand and then lightly gripping a toothpaste cap. because of the conical shape of the cap, any over gripping led to it flying out of my hand. while doing this, i was also careful in making sure my other digits weren't flaring out. sounds easy, but this took me 2 years to master after practicing EVERY night. btw, the smaller the object, the more difficult it was for me. i started with the biggest cap i could find and worked my way to the smaller size.
